The outcome is not immediate catastrophe. It is gradual accumulation of risk—visible only in retrospect after adverse events.

  • Robust safety culture

Recognizing these four laws does not eliminate error. It allows institutions to design against it.

For clinicians and administrators alike, the practical question is not whether these forces operate in your organization. It is where—and whether they are being actively counterbalanced.
